Completing comprehensive nursing continence assessment in your home or preferred location at a time set up by our bookings

Assessments are conducted between 4pm and 6 pm or at an agreed time

NDIS continence consumables can be assessed and set up as part of your NDIS care plan.

Service bookings, Payment requests can be completed if agency managed, or emailing your plan manager

TAC/WORKSAFE Continence ASSESSMENTS

State Governement and Federal Government funding schemes including CAPS and SWEP

Specialising in Home Care Package referrals from Level 3 and 4 Packages

TAC/WORKSAFE Continence ASSESSMENTS

TAC and Work safe Continence Nursing Assessments can be completed. please allow 5 hours for the Assessment plus travel costs

TeleHealth Consultations

SWEP Prescriptions

SWEP Prescriptions and Eligibility Forms can now be completed via TeleHealth Consultations. Allow 1 hour for the TeleHealth Consultation

SWEP Prescription Renewals 12 monthly

SWEP Prescriptions need to completed 12 monthly by a Credentialed Continence Nurse. Allow 30 minutes - 60 minutes for the TeleHealth Consultation

NDIS Continence Assessments via TeleHealth

Eastwood Continence is able to complete NDIS Continence Assessments in Tasmania via TeleHealth Consultations. Other States in Australia can also be accommodated

What you need to provide

A medical history provided by your GP/Specialist

For the assessment, provide a medical history letter provided by your GP or Specialist

Home Care Package Cost $171.37 per hour for a minimum for 2 hours plus travel

NDIS Continence Assessments Cost $171.37 per hour plus travel ( allow for 6 hours in your NDIS Care Plans to complete the assessment)

Telehealth Consultation $171.63 per hour

Travel is charged at $110 per hour

Accounts all settled on day of assessment

Payment via EFT or cheque only
